<p>In a&#xa0;retrospective analysis, we investigated the predictive value of automated stone measurements (AM) in kidney stone patients who underwent extracorporeal shock wave lithotripsy (ESWL) between 2016 and 2022, meeting specific inclusion criteria. In 197 renal units the success rate was 44.2%. Pixel-by-pixel automated measurements (AM) showed an average HU (Hounsfield units) of 534, compared to 937 in manual measurements (HM) (<i>p</i> &lt; 0.001). Univariate analysis indicated that lower BMI, smaller diameter (in both AM and HM), and lower skewness (indicating less homogeneity) were associated with ESWL success. HU values were not significant predictors. AM and HM differed significantly but showed similar predictive values in multivariate models. The relevance of HU skewness offers new possibilities for optimizing patient selection for ESWL.</p>
