Prevalence of prediabetes and type-2 diabetes risk factors using the ADA risk tool: A cross-sectional study in an indian tertiary care setting

Prediabetes represents a transitional state with elevated risk for type 2 diabetes mellitus (T2DM). Early identification is critical in high-burden countries like India.
ObjectiveThis study aimed to determine the prevalence of prediabetes and distribution of T2DM risk factors among adult patients visiting a tertiary care hospital.
MethodsA cross-sectional study of 315 adults was conducted at a tertiary care hospital. Participants were assessed using the ADA/CDC risk tool. Laboratory parameters (FPG, HbA1c) were measured. A score ≥ 5 was defined as high risk. Associations were tested using chi-square and t-tests.
ResultsData were gathered from 315 participants; 171 (54.3%) were classified as high risk for developing T2DM. Risk was significantly higher in men than women (69.0% vs. 44.4%, p = 0.003), and it rose steadily with age, with all individuals over 60 years identified as high risk. Hypertension was the strongest predictor, with 89.1% (95% CI: 82.3%–93.8%) of hypertensive participants at risk, followed by obesity, where nearly seven in ten (69.6%, 95% CI: 57.2%–80.1%) were at risk. These findings indicate that male gender, older age, hypertension, and obesity were the major contributors to T2DM risk in this population, highlighting the need for early and targeted preventive measures.
ConclusionThe ADA/CDC tool effectively identified high T2DM risk in this Indian hospital-based population. Hypertension, obesity, and older age were the strongest predictors. Findings underscore the need for community-based validation and preventive interventions.
