Objective <p>The Maha Kumbh 2025 in Prayagraj presented an unprecedented opportunity to conduct large-scale health screenings among a diverse population from across India. This study aimed to estimate the prevalence of diabetes, hypertension, and cardiovascular diseases among attendees, while assessing medication adherence patterns during mass religious gatherings.</p> Methods <p>A prospective, cross-sectional study was conducted during the Maha Kumbh 2025 (February 2025) among adult participants aged 18&#xa0;years and older. Data collection included demographic information, anthropometric measurements, point-of-care testing for random blood glucose and blood pressure, and assessment of medication adherence. Statistical analysis was performed using Python software with appropriate tests for categorical and continuous variables.</p> Results <p>A total of 26,577 subjects were screened with a mean age of 48.6 (± 14.9) years. The study population comprised 66.6% males and 63.4% rural residents. The prevalence of major non-communicable diseases (NCD) was substantial: 25.5% had diabetes, 16.1% had hypertension, and 5.2% had cardiovascular disease. Overall, 29.9% of participants had at least one chronic condition. Among those with diabetes, 52.8% also had hypertension. The mean BMI was 25.3 (± 4.5) kg/m<sup>2</sup>, with 71.4% classified as overweight or obese. Participants with diabetes were significantly older (53.0 vs 47.1&#xa0;years, <i>p</i> &lt; 0.001), more likely to be from rural areas (73.8% vs 59.9%, <i>p</i> &lt; 0.001), and had higher rates of alcohol (38.4% vs 13.8%, <i>p</i> &lt; 0.001) and tobacco consumption (27.9% vs 14.3%, <i>p</i> &lt; 0.001). Medication non-adherence was concerning, with 28.6% of individuals with diabetes and 43.6% of individuals with hypertension having forgotten their medications.</p> Conclusion <p>This large-scale screening program revealed a significant burden of NCDs among Maha Kumbh attendees, with high rates of diabetes, hypertension, and obesity. The study demonstrates the feasibility and importance of implementing health screenings during mass religious gatherings, while highlighting critical challenges in medication adherence during travel. These findings underscore the need for targeted health interventions and comprehensive medication management support during such events.</p>
