Objective <p>To systematically evaluate and compare the glycemic and metabolic profiles between fibrocalculous pancreatic diabetes (FCPD) and type II diabetes mellitus (T2DM).</p> Methods <p>The protocol was registered on PROSPERO (CRD42022384560). PubMed and Embase were searched using predetermined MeSH terms and keywords. A title + abstract screening followed by full-text screening was done for resultant selected articles using predetermined criteria. The NHLBI quality assessment tool was used. Meta-analysis was performed to compare the BMI, glycemic parameters such as FBS, PPBS, and HbA1c, and the metabolic profile which included HDL, LDL, triglycerides, and total cholesterol, between FCPD and T2DM.</p> Results <p>FCPD patients exhibited significantly higher levels of FBS (<i>p</i> &lt; 0.02), PPBS (<i>p</i> &lt; 0.01), and HbA1c (<i>p</i> &lt; 0.00001) compared to T2DM patients with a mean difference of 17.90, 38.88, and 0.89 respectively. Conversely, T2DM patients had higher levels of LDL (<i>p</i> &lt; 0.001), VLDL (<i>p</i> &lt; 0.001), and TG (<i>p</i> &lt; 0.001). These findings suggest distinct metabolic profiles between FCPD and T2DM patients.</p> Conclusion <p>Our study findings demonstrate that the glycemic parameters and metabolic profiles of patients with T2DM versus FCPD vary considerably where patients of FCPD have more severe hyperglycemia and poor glycemic control requiring rigorous strategies for glycemic control in contrast to patients of T2DM who are more prone for metabolic dysregulation.</p>
