Molecular Characterization and Relative Abundance of Biofilm-Producing Carbapenemase-Producing Enterobacteriaceae (CPE) or Carbapenemase-Producing Organisms (CPOs) of Clinical Origin in the Jazan, Saudi Arabia

The synergistic interplay between biofilm production and carbapenem-resistance-associated (CRA) genes exacerbates the therapeutic challenges of device-related CPE/CPO infections. This research aimed to characterize biofilm-producing CPE/CPO. A total of n = 271 specimens was analyzed by GeneXpert Carba-R Assay and VITEK system for strain characterization, with biofilm production assessed through the Congo Red Agar assay. CPE/CPO strains constituted 12.64% (n = 23) of isolates. The relative abundances were: Klebsiella pneumoniae (38.5%, n = 70), Escherichia coli (30.77%, n = 56), and Pseudomonas aeruginosa (14.8%, n = 27). Least identified were non-CPE/CPO strains Moraxella catarrhalis and Citrobacter werkmanii (0.5%, n = 1 each). Klebsiella pneumoniae was most frequent in sputum and blood, while Escherichia coli and Pseudomonas aeruginosa predominated in urine and wound swabs, respectively. CRA genes such as OXA-48 (48%), NDM (26%), and a combination of these two genes were predominantly found in the CPE/CPO strains. The highly frequent CRA genes in Klebsiella pneumoniae were OXA-48 (39.13%) and NDM (13.05%). E. coli, Pseudomonas aeruginosa and Acinetobacter baumannii harbor only one of the CRE genes. E. coli harbored the OXA-48 gene; however, Pseudomonas aeruginosa and Acinetobacter baumannii possessed NDM resistance genes. Most (82.6%) of the CPE/CPO strains were moderate to strong biofilm producers. 94.11% of the K. pneumoniae were moderate-strong biofilm producers. OXA-48 and NDM CRA genes in biofilm-producing CPE/CPO necessitate strengthening surveillance and effective infection control policies.
