<p>Laparoscopic gastrostomy represents a valuable alternative to percutaneous endoscopic gastrostomy (PEG), particularly in cases of contraindication or technical failure. We describe a novel simplified technique consisting of a double barbed pursestring suture performed laparoscopically. Between December 2023 and December 2024, eight patients with swallowing disorders due to neurological conditions underwent this procedure. The mean operative time was 27 minutes (range 20–35), and enteral feeding was initiated on the day of surgery in all cases. No intraoperative or postoperative complications were observed, and the mean hospital stay was one day. At a mean follow-up of 180 days (range 90–360), all gastrostomies remained functional, and none required replacement or hospital readmission. These results suggest that laparoscopic gastrostomy with double barbed purse-string suture is a safe, fast, and reproducible technique, representing an excellent option when PEG is not feasible.</p>
