Background <p>Doppler ultrasonography of the Ophthalmic Artery (OA), due to its embryological and physiological relationship to the cerebral vasculature, offers a non-invasive method for assessing vascular alterations in PE. This study aimed to compare OA Doppler parameters between normotensive and pre-eclamptic pregnant women.</p> Methods <p>A cross-sectional study was conducted involving 102 pregnant women (72 normotensive, 30 with pre-eclampsia). OA Doppler sonography was performed using a Samsung DC N3 USG machine. Parameters evaluated included first peak systolic velocity (PSV1), second peak systolic velocity (PSV2), end-diastolic velocity (EDV), pulsatility index (PI), and the PSV2/PSV1 ratio. Statistical analysis was performed using independent sample t-tests.</p> Results <p>Significant hemodynamic differences were observed between the groups. Women with pre-eclampsia demonstrated significantly lower mean PSV1 (<i>p</i> = 0.016) and PI values (<i>p</i> &lt; 0.001), while mean PSV2 (<i>p</i> = 0.010), EDV, and the PSV2/PSV1 ratio (<i>p</i> &lt; 0.001) were significantly higher compared with normotensive controls. These findings indicate altered Ophthalmic Artery hemodynamics in pre-eclamptic pregnancies.</p> Conclusion <p>Pre-eclampsia is associated with characteristic alterations in Ophthalmic Artery Doppler indices, particularly reduced PI and increased PSV2/PSV1 ratio, reflecting changes in vascular resistance and orbital blood flow. OA Doppler sonography may serve as a useful non-invasive adjunct for the assessment and monitoring of vascular changes in pre-eclampsia.</p>
