Maternal Risk Factor Stratification and Outcomes of Births at a Tertiary Perinatal Referral Centre Network in India: An Observational Study

An overview of risk categorization in pregnancy, percentage of low-risk women turning high-risk, the trends of age at conception, Body Mass Index (BMI), prevalence of single/multiple medical disorders in pregnancy in past 11 years.
Study DesignA cross-sectional observational hospital-based study between January 2013 and December 2023 at a tertiary perinatal referral centre in India. The information from 102,683 births analysed in this study was extracted from the electronic medical record database and organized into a unified excel spreadsheet.
ResultsOf the total 99,467 women, 65.05% were low-risk, and 34.95% were high-risk at the time of booking. Twenty four percent women remained as low-risk and 41.49% low-risk women became high-risk. The common reasons for low-risk women turning high-risk were Gestational Diabetes Mellitus (GDM) (36.3%), Hypertension (HTN) in pregnancy (17.7%) and thyroid disorders (14.9%). The percentage of women conceiving after 35 years was 5.7 in 2013 and 11.5 in 2023 and after 40 years was 0.6 in 2013 and 1.3 in 2023 which doubled. The prevalence of systemic disorders complicating pregnancy increased from 51.8% in 2013 to 72.9% in 2023. Of these, the incidence of HTN complicating pregnancy increased from 11.5% to 14.3%, diabetes mellitus (DM) complicating pregnancy from 20.5% to 30.5%, and thyroid disorders from 19.4% to 27.5% over past 11 years.
ConclusionRisk categorization is essential throughout pregnancy to identify women at high-risk and streamline the care. Extrapolating changing trends in maternal characteristics help in planning pregnancy and acquiring healthy lifestyle.
