Sentinel Lymph Node Biopsy in Endometrial Carcinoma: A Prospective Feasibility Study

Sentinel lymph nodes are the first nodes receiving drainage of tumor cells from primary tumors. They provide important prognostic information about the nodal status of a malignancy. The role of sentinel lymph node in endometrial cancer has been justified in many trials but the feasibility still needs to be proven at any center that aims to adopt the technique. Thus, this study was undertaken with the aims to calculate the detection rate and diagnostic accuracy (sensitivity, specificity, negative and positive predictive value) of sentinel lymph node biopsy in endometrial carcinoma by using indocyanine green dye.
MethodologyProspective study was conducted at our hospital over a period of 2 years. Women with endometrial carcinoma with tumor confined to the uterus and no metastasis in imaging studies or no obvious extra-uterine disease on exploration were included in the study. All patients underwent staging surgery with SLN procedure that was conducted as per the latest NCCN guidelines.
Results and AnalysisA total of 58 patients with a mean age of 58.98 years were evaluated. Bilateral detection rate of sentinel lymph nodes was 90% with a sensitivity of 85.7% and specificity of 100%. The negative predictive value was 98%.
ConclusionThe diagnostic accuracy of sentinel lymph node detection in Carcinoma endometrium confined to uterus on pre-operative imaging has a sensitivity of 85.7% and a negative predictive value of 98%. Thus, sentinel lymph node mapping is feasible at our institute.
