Background <p>Vertebral hemangiomas are mostly benign vascular lesions frequently found incidentally. They may exhibit aggressive behavior during pregnancy because of hormonal and hemodynamic changes.</p> Case Presentation <p>We present a case of a 22-year-old woman who developed spastic paraplegia in the third trimester due to an aggressive vertebral hemangioma compressing the spinal cord. Timely MRI and coordinated multidisciplinary care resulted in a successful vaginal birth followed by neurosurgical decompression and spinal stabilization. The patient demonstrated excellent recovery.</p> Conclusion <p>This case underscores the need for prompt decision-making, imaging, tailored delivery planning, and postpartum neurosurgical interventions. It also adds to the scarce literature on the obstetric-neurosurgical care pathways for vertebral hemangiomas during pregnancy.</p>

Background

Vertebral hemangiomas are mostly benign vascular lesions frequently found incidentally. They may exhibit aggressive behavior during pregnancy because of hormonal and hemodynamic changes.

Case Presentation

We present a case of a 22-year-old woman who developed spastic paraplegia in the third trimester due to an aggressive vertebral hemangioma compressing the spinal cord.
