<p>Necrotizing fasciitis is a rare, aggressive surgical infection with high rates of morbidity and mortality. A 33&#xa0;years old primigravida with normal antenatal course had full-term vacuum delivery with episiotomy. On 3rd postpartum day, patient developed extreme pain at the episiotomy site. On examination, there was swelling and induration of left labia extending to buttocks. MRI done was suggestive of cellulitis with extensive subcutaneous edema. A multidisciplinary management with immediate aggressive debridement, broad spectrum antibiotics, and intensive care was adopted. Patient had a long hospital stay of 60&#xa0;days where she underwent 20 sessions of hyperbaric oxygen therapy (HBOT) and 16 surgical procedures along with multiple blood and blood products transfusions. This case affirms the potential role of multidisciplinary approach with early diagnosis and aggressive management of such rare but fatal infection.</p>
