Shining Light on the Shadows: Unraveling Primary Malignant Melanoma of the Uterine Cervix—A Rare Case Report and Therapeutic Odyssey

Melanomas are malignancies that arise from melanocytes; skin being the common site. It can rarely occur in the mucosa of the urogenital tract, vagina. Primary malignant melanoma of the cervix is extremely rare with only a few cases (100) reported in the worldwide literature.
Case ReportWe report a case of a 75-year-old woman, para 5, presented with a complaint of blood-tinged white discharge for 2 years. The diagnosis made by histopathologic examination of cervical biopsy and immunohistochemistry for human melanoma black 45(HMB45) and Melan A. She was BRAF V600 wild type and was referred to a medical oncologist.
Discussion & ConclusionMost of the patients present with vaginal/ post coital bleeding. The diagnosis remains by histopathological examination of colposcopy guided cervical biopsy and immunohistochemical staining. Radiological investigations such as abdomenopelvic magnetic resonance image scan (MRI) and whole-body positron emission tomography (PET/CT) used for the staging. No standardized protocols or specific treatment guidelines available for this rare cancer, so treatment for each case should be individualised and done by a multidisciplinary team. Management is case based, usually surgical excision followed by focal external beam or intracavitary radiotherapy (RT). Adjuvant treatment measures like chemotherapy, RT, and immunotherapy are reserved cases of metastatic disease and local recurrences.
