Introduction <p>Oral cavity squamous cell carcinoma (OCSCC) encompasses distinct subsites with differing oncological behaviors. This study aimed to compare survival outcomes between Tongue-Floor of the mouth Squamous cell carcinoma (TFSCC) and gingivobuccal squamous cell carcinoma (GBSCC) treated with surgery and appropriate adjuvant treatment.</p> Methods <p>A retrospective, cohort study analyzed 778 patients with OCSCC who underwent surgery with neck dissection and appropriate adjuvant therapy. Primary outcomes were five-year overall survival (OS) and recurrence-free survival (RFS). Key covariates included patient demographics, tumor characteristics, treatment modalities, and pathological factors. Propensity score matching (PSM) adjusted for age, sex, tumor stage, and nodal status. Kaplan-Meier survival curves and log-rank tests assessed survival differences before and after matching.</p> Results <p>Unmatched five-year OS was significantly higher in TFSCC (70%) than GBSCC (55%, <i>p</i> = 0.0149). Similarly, RFS favored TFSCC (61.6%) over GBSCC (48.7%, <i>p</i> = 0.0198). However, after matching (289 per group), survival differences were not significant (OS: 67.3% vs. 61.6%, <i>p</i> = 0.187; RFS: 55% vs. 48.7%, <i>p</i> = 0.12).</p> Conclusions <p>The study showed that TFSCC had better survival outcomes than GBSCC before matched pair analysis. But this difference disappeared after the matching. The study needs to be validated on multicentric datasets.</p>
