Background <p>There are multiple factors responsible for recurrent sarcoma including patient age, tumor size, site, depth, and histological subtype, resection with adequate margins, tumor rupture or incomplete resection, and conduction of radiation or chemotherapy.</p> Patients and Methods <p>This study included 213 patients with recurrent soft tissue sarcoma in the extremities, trunk, head and neck, genitourinary and abdomen. We evaluated the pattern of recurrence in these patients either local, nodal and distant recurrence and treatment including surgical resection, chemotherapy or radiotherapy.</p> Results <p>The age of patients was 44.77 ± 17.18 years, and male to female percentage was (51.2% vs. 48.8%). The common location of recurrent soft tissue sarcoma was in the limbs in 128 patients and the abdomen in 27 patients. The primary tumors sized 10.81 ± 11.52&#xa0;cm, and the depth of invasion was 5.67 ± 4.14&#xa0;cm. The grade (G) of primary tumors was GI, GII, GIII in 61%, 8.5%, 30.5%, respectively. The pattern of recurrence was local, nodal and distant in 201, 36, and 56 patients respectively.</p> Conclusion <p>Soft tissue sarcoma are aggressive tumors that need multidisciplinary management and should be removed with adequate safety margins as local recurrence commonly occurs in tumors not removed with free margins. Regular follow up is needed up to 10 years and still recurrence is inevitable.</p>
