Clinical Breast Examination for Early Detection of Breast Cancer in Women over 40 Years: A Cross-Sectional Study in a Tertiary Care OPD Setting in India

In low-resource settings with limited access to mammography, clinical breast examination (CBE) by trained personnel may offer a viable early detection strategy for breast cancer [1, 9]. This study aims to assess the diagnostic accuracy of CBE in detecting breast cancer among women ≥40 years and describe the clinical spectrum of breast complaints in a tertiary care OPD. A cross-sectional study was conducted among 245 women aged 40–90 years attending the surgery OPD between January 2023 and December 2024. CBE was performed by trained surgeons, with abnormal findings referred for mammography and/or FNAC [10, 12]. Breast lumps were detected in 6.5% of participants; 3.7% reported nipple discharge. Histopathological confirmation revealed a breast cancer prevalence of 4.1%. Sensitivity and specificity of CBE for palpable lumps were 90% and 97%, respectively (PPV 56%, NPV 99.6%). Including any abnormal CBE finding (lump/discharge/skin change) yielded 100% sensitivity. CBE conducted by trained personnel in routine OPD workflows is a highly sensitive and low-cost screening method for early breast cancer detection. Integration into standard surgical practice in resource-limited settings is strongly recommended [7, 15].
