<p>Neck dissection (ND) is an integral part of multimodal management of head and neck cancer (HNC). In the neck, four patterns of anatomical relationship between spinal accessory nerve (SAN) and internal jugular vein (IJV) were described in literature. A rare pattern of SAN traversing through a fenestration in the upper part of IJV is described in this case report. The objective is to familiarise surgeons about the various anatomical relations of SAN and IJV to avoid iatrogenic injury during surgery.</p>
