<p>To assess the prognostic significance of NLR, PLR, and ANC in predicting 5-year overall survival (OS) in acral melanoma (AM) patients. This retrospective study examined AM patients who underwent wild local excision (WLE) alone or with sentinel node biopsy (SNB) from 2001 to 2015. The primary outcome was 5-year OS. Optimal cutoff points for biomarkers were determined, and statistical analyses included Kaplan–Meier curves, log-rank tests, and Cox regression. Among 394 patients, mean age 65.3&#xa0;years; 58.1% female, higher 5-year OS rates were observed with NLR &lt; 1.8, PLR &lt; 120, and ANC &lt; 4,000. NLR was significantly associated with OS (hazard ratio: 1.55; 95% CI: 1.01–2.04). NLR is a promising, though not yet fully validated, prognostic marker in AM patients.</p>
