<p>Sacrectomy presents significant anesthetic challenges due to extensive surgical resection, prolonged operative duration, potential for substantial blood loss, hemodynamic instability, metabolic disturbances, positioning-related challenges, and pain management. We report an interesting case of a giant cell tumor (GCT) of sacrum posted for sacrectomy presenting with unique anesthetic challenges. Due to rarity of the procedure, there is limited literature on its anesthetic management. This case report aims to provide valuable insights into optimizing anesthetic strategies enhance recovery and reduce perioperative morbidity. The integration of enhanced recovery after surgery (ERAS) principles has further refined anesthesia strategies mitigating homeostatic disturbances, attenuating the stress response and reducing postoperative complications.</p>
