<p>Perinephric space is a conical territory lying within the retroperitoneum and contains the kidney, renal vessels, ureters, adrenal gland along with fat and supporting connective tissue. A multitude of diseases can involve this relatively small compartment and can arise from the perinephric space itself or more commonly as an extension from organs viscera within the space (e.g. kidney, adrenals) and from the retroperitoneum. Cross-sectional imaging with CT and MRI forms the cornerstone in radiological evaluation of the pathologies affecting this space. After ascertaining the origin, identification of the morphological appearance further allows the radiologist to narrow the differential diagnoses. This review aims to illustrate the various neoplastic and non-neoplastic entities occurring in the perinephric space and describe a systematic approach for formulating the correct diagnosis/differential diagnoses.</p>
