[68 Ga]Ga-DOTATATE in Identifying Giant Cell Tumor Causing Tumor-Induced Osteomalacia

A 43-year-old male with progressive musculoskeletal pain and recurrent fractures was misdiagnosed with ankylosing spondylitis due to HLA-B27 positivity. Metabolic evaluation confirmed tumor-induced osteomalacia (TIO). [68 Ga]Ga-DOTATATE PET/CT localized the culprit lesion to a right iliac mass (SUV max 26.6), distinguishing it from multiple insufficiency fractures exhibiting only faint uptake. Histopathology confirmed a rare giant cell tumor of soft tissue. Following excision, phosphate levels normalized with significant functional recovery. This case underscores the specificity of [68 Ga]Ga-DOTATATE in TIO and warns against diagnostic "red herrings" like incidental HLA-B27.
