Mendelian randomization analysis of immune cell traits and oral microbiota in nasopharyngeal carcinoma during radiotherapy

To explore the relationship between oral microbiota and the degree of oral mucosal injury during radiotherapy for nasopharyngeal carcinoma (NPC).
MethodsFrom Jun 2022 to Apr 2023, 40 NPC patients (research group) with oral mucosal injury who received radiotherapy and their healthy family members 40 cases (control group) in department of head and neck radiotherapy, Meizhou People’s Hospital were selected as the research population, to compare the diversity of oral mucosal flora alpha between the two groups. Subsequently, research group were graded as either mild (RTOG 1-II) or severe oral mucosal injury (RTOG III-IV) according to the RTOG classification. Samples of oral pharyngeal swab samples of patients were obtained for operational taxonomic units (OTUs) clustering, and the abundance table was obtained. Additionally, Mendelian randomization analysis examined genetic associations between immune cell traits and head and neck squamous cell carcinoma (HNSCC) risk using publicly available GWAS data.
ResultsCompared to the control group, NPC patients showed significantly lower α-diversity of oral microbiota (lower Chao1, PD, and Faith’s PD indices; p < 0.05). Patients with severe oral mucosal injury demonstrated reduced microbial diversity compared to those with mild injury. The relative abundance of Actinobacteria, Firmicutes, and Fusobacteria was higher in patients with severe injury, while Bacteroidetes, Patescibacteria, and Spirochaetes were lower. Mendelian randomization analysis identified specific immune cell traits associated with HNSCC risk, including potential protective effects of CD28 + CD8 + T cells and potential risk associations with CD24 + CD27 + B cells.
ConclusionThere is a close relationship between oral mucosal injury during radiotherapy of NPC patients and oral microflora imbalance. In the future, we can prevent the occurrence of oral mucosal injury by regulating oral microbiota, so as to provide a more reliable guarantee of good prognosis for patients.
