<p>This case report describes a recurrent giant subplatysmal neck lipoma in a 67-year-old female presenting with progressive enlargement and displacement of adjacent vital deep neck space structures. Imaging demonstrated a well-circumscribed fat-density lesion without features suggestive of malignancy. Complete surgical excision was performed with careful preservation of adjacent neurovascular structures. Histopathological examination confirmed a benign lipoma. The case highlights the importance of thorough reassessment of recurrent neck masses, appropriate imaging evaluation, and meticulous surgical technique to prevent recurrence. Although rare, large recurrent subplatysmal lipomas may mimic aggressive pathology and require careful management.</p>
