<p>A 64-year-old woman with a history of breast cancer presented with limited mouthopening (0.7 cm) and left cheek pain. Initial symptoms included numbness in the lower left jaw, leading to tooth extractions, but symptoms persisted. Clinical examination revealed swelling in the left masseter region. Imaging (CBCT and ultrasonography) identified a suspicious mandibular lesion and hypoechoic areas in the masseter muscle. Biopsy confirmed malignancy, and PET-CT revealed metastatic lesions in the lungs and liver, diagnosed as metastatic squamous cell carcinoma. Despite targeted therapy and radiation, the patient succumbed to the disease eight months after presentation. This case underscores the diagnostic challenges of malignancies mimicking temporomandibular disorders.</p>
