Cervico Facial Bull Horn Penetrating Injury: An Unusual Presentation with Orocutaneous Fistula—Case Report

Penetrating bullhorn injuries to the cervical and oral regions are rare but pose significant challenges. We present a case of a 68-year-old male with an Oro-cutaneous fistula and mandibular fracture following a bullhorn injury to Zone 2 of the neck. Initial mismanagement led to infection and tissue necrosis. Definitive surgery was performed 42 days later, involving removing infected hardware, mandibular fixation, and fistula closure with a Pectoralis Major Myo-cutaneous (PMMC) flap. The patient recovered with restored mouth function and well-healed fractures. This case highlights the need for prompt multidisciplinary care to prevent complications in such rare injuries.
