<p>This article presents a rare case of a 22-year-old female with an epidermoid cyst in the retroauricular region, an uncommon location for such lesions. The cyst had been progressively enlarging over the past year, with no history of trauma, ear piercing, or familial predisposition. Clinical evaluation revealed a well-defined, soft, cystic, non-tender mass with limited mobility, while the overlying skin appeared normal and non-adherent. Surgical excision was performed, and histopathological evaluation confirmed an epidermoid cyst characterized by stratified squamous epithelium, keratinous debris, and a fibrous capsule. The distinctiveness of this case lies in its atypical location and the absence of common predisposing factors. Complete surgical excision proved both definitive and curative. This case emphasizes the importance of histopathological confirmation to distinguish rare postauricular cysts from mimicking conditions such as lymphadenopathy or abscesses, thereby guiding effective surgical management.</p>
