Aim <p>The aim of this study was to analyze the epidemiological profile of patients treated with orthognathic surgery at a private clinical practice in Munich, Germany between January 2022 and June 2024.</p> Material and Methods <p>A retrospective chart review was conducted. Pearson’s chi-square test was applied to assess frequencies, with the significance level set at 5%.</p> Results <p>This study included 383 patients who underwent a total of 394 orthognathic procedures, excluding genioplasty or surgery performed only to remove fixation devices. Mean patient age was 31.34 years old (age range 25–34 years old) with a female predominance (54.3%). The most prevalent surgical procedure was bilateral sagittal split osteotomy (BSSO) + Le Fort I osteotomy (<i>n</i> = 156), followed by BSSO only (<i>n</i> = 119), and 2-piece surgically assisted palatal rapid expansion (<i>n</i> = 68). The correlation between surgery and sex was not statistically significant (<i>p</i> &gt; 0.05), but between age and sex, age and surgical procedure were statistically significant (<i>p</i> &lt; 0.05). Only two patients (0.5%) occurred surgical complication, pseudoarthrosis and plate fracture.</p> Conclusion <p>Bimaxillary osteotomy was the most prevalent surgical approach, and the mandible was the most affected bone. These findings indicate a higher frequency of orthognathic surgery among female and younger patients, with a low complication rate, supporting the safety and efficacy of the procedure.</p>
