<p>The median lower lip cleft, characterized by a congenital anomaly of the lower lip, represents a unique intersection of developmental biology and clinical practice. Occurring as a rare form of clefting, it typically manifests as a notch or fissure in the midline of the lip, with varying degrees of severity impacting both cosmetic appearance and functional capabilities. This case report highlights the rare occurrence of non-syndromes boy with median lower lip clefts, their embryological origins, and the surgical approach taken. There were no associated anomalies, and surgical correction was performed using a “V”-shaped incision, achieving favourable functional and aesthetic outcomes. Further documentation of such cases will contribute to enhancing the understanding and management of similar craniofacial anomalies.</p>
