Objective <p>Simple bone cyst (SBC) of the jaw is a type of pseudocyst that often difficult to distinguish from other jaw bone cyst. Hence, we intended to investigate the clinical and radiographic diagnostic characteristics of SBC.</p> Methods <p>Patients with SBC who underwent surgery at Peking University Hospital of Stomatology between 2008 and 2023 were examined and contrasted with matched cases of odontogenic keratocyst (OKC) and apical cyst (APC).</p> Results <p>SBC predominantly occurred in adolescents aged 11–20, comprising 73.2% of cases. Female was more prevelant than male (M:F = 1:1.55). Majority (80.4%) of patients were asymptomatic. The size of SBC was relatively small (maximum diameter rarely surpasses 4.0&#xa0;cm), and 58.9% of the lesions were located in the anterior region of the mandible. Among these characteristics, the age of onset, history of trauma, and clinical symptom differ significantly from those of OKC and APC (<i>P</i> &lt; 0.05). Beside, the degree of bone expansion of SBC was smaller than that of OKC, and it usually presented distinctive cortical bone resorption without bone expansion. Most of the SBC lesions (70.9%) were identified empty cavities intraoperatively, and no epithelial lining with bone debris were found in all cases. Curettage surgery was performed for all patients, and SBC showed a significantly slower bone healing rate compared with OKC and APC (<i>P</i> &lt; 0.001).</p> Conclusion <p>SBC is usually an asymptomatic lesion occurs in adolescents, and often discovered in the anterior region of the mandible. The healing rate of SBC after surgery is relatively slower than other odontogenic cyst.</p>
