Objective <p>This retrospective study evaluated the impact of surgical extraction of partially impacted mesioangular type I lower third molars on the periodontal health of adjacent second molars.</p> Methods <p>A retrospective analysis of electronic health records from 200 patients treated at the Dental School of Tehran Islamic Azad University (April–August 2022) was conducted. After applying inclusion criteria (18–25&#xa0;years old, standardized surgical protocol, pre/post-operative periodontal assessments), 36 patients were included. Periodontal indices—plaque index (PI), probing pocket depth (PPD), clinical attachment loss (CAL), and bleeding on probing (BOP)—were compared at baseline and 4&#xa0;months post-surgery.</p> Results <p>Statistically significant differences were observed in PI (1.72 ± 0.88 vs. 0.89 ± 0.74, <i>p</i> &lt; 0.001), PPD (3.15 ± 0.68&#xa0;mm vs. 3.09 ± 0.69&#xa0;mm, <i>p</i> = 0.002), CAL (1.14 ± 0.18&#xa0;mm vs. 1.13 ± 0.20&#xa0;mm, <i>p</i> = 0.003), and BOP prevalence (61.1% vs. 27.8%, <i>p</i> = 0.002).</p> Conclusion <p>Surgical extraction of mesioangular third molars is associated with improved periodontal outcomes in adjacent second molars, particularly in younger patients. Retrospective findings suggest that standardized protocols and early intervention may optimize results.</p>

Objective
