<p>Hemangioma being a benign vascular neoplasm frequently presents in the soft tissues. Less than 1% of the hemangiomas comprise the intraosseous type, having predominantly congenital origin. They have extremely rare presentation in the mandible, and literature search could not decipher many articles on intraosseous hemangioma (IOH) when it comes to the mandible. Enormous vasculature network and indistinguishable appearance clinically as well as radiographically with other cysts/tumors makes its diagnosis as well as management challenging. Hence, we present an unusually complex and rare case of a massive IOH of the mandible in a 13-year child, focusing on its differential diagnosis, pre-operative evaluation/screening, step-by-step management approach, and its long-term treatment plan. Management of IOH vary considering the patient’s age, size, location and flow characteristics, and functional compromise. It can be either conservative or surgical, including sclerotherapy embolization, curettage, and surgical resection and reconstruction. Recurrence rate of IOH is also not well-documented. Thus, the ideal age/time post-resection for bony reconstruction is arguable. Nonetheless, this case report has gathered the insights into this rare entity, highlighting the advantages and disadvantages of each treatment, which can further help in pertinent pre-operative evaluation and management tailored as per the patient needs.</p>
