<p>Neuroendocrine carcinoma (NEC) is a tumor composed of cells from both the nervous and endocrine systems, capable of releasing neuropeptides into the bloodstream. Despite their rarity, their behavior in the head and neck location does not significantly differ from their behavior in more common sites of origin. In this article, we present a 54-year-old female who presented to us with complaints of a large swelling on the left side of her neck, accompanied by pain and restricted mobility of the head. Subsequently, a biopsy was conducted, and the histopathological results indicated small-cell neuroendocrine carcinoma. In this report, we have outlined the chosen diagnostic and treatment strategy, providing comprehensive insights into this uncommon giant tumor. To our knowledge, this is the first reported case involving such a substantial tumor mass (733&#xa0;gm) in the supraclavicular region in India.</p>
