Effects of Age, Sex, Operating Time, and Blood Loss on Changes in Peripheral White Blood Cell Count Following Major Oral and Maxillofacial Surgery

To determine whether the changes in neutrophil, lymphocyte, and total WBC counts among patients who had major oral and maxillofacial (OMF) surgical procedures were influenced by sex, age, operating time and blood loss.
DesignA prospective observational study.
SettingOral and Maxillofacial Surgery Department of the University of Benin Teaching Hospital, Nigeria.
ParticipantsPatients requiring OMF surgeries under general anesthesia were consecutively selected. Those 50 years and above and those who required combined surgery were excluded.
InterventionsBlood samples were collected preoperatively, and postoperatively (immediate, 24-h, 72-h, and 120-h periods) and analyzed using automated blood cell counter (Sysmex KX-21).
Main Outcome MeasuresData were analyzed with SPSS version 20.0. A P-value of < 0.5 was considered significant. The difference between means of measurement was analyzed using a one-way analysis of variance (ANOVA) test while the possible effect of confounding factors was also tested using multiple linear regression techniques.
Results72 participants, with a mean age of 15.2 ± 1.7 years, were studied. Blood loss of up to 50 ml in the young patients evoked a significant increase in the WBC/neutrophil counts. Surgeries lasting over 60 min also demonstrated considerable effects. Sex showed no statistical difference. However, age showed statistical significance in the total WBC and neutrophil counts but not in the lymphocytes.
ConclusionsAge, blood loss, and operating time were important causes of changes in WBC count, except sex. However, controlling for these variables, we found age to be a significant predictor of the observed changes in the WBC count.
