Surgical and Anesthetic Considerations in the Management of Ossifying Fibroma of the Mandible with Congenital Factor VII Deficiency: A Case Report and Literature Review

To present a rare case of congenital factor VII (FVII) deficiency in a 15-year-old male with ossifying fibroma of the mandible and outlining the surgical and anesthetic management strategies.

The patient underwent routine investigation for prolonged prothrombin time (PT) and was diagnosed with congenital FVII deficiency. Clearance for surgery was obtained from the anesthesia and hematology departments. Four units of fresh frozen plasma were administered intraoperatively to manage coagulation.

During surgery, minimal bleeding occurred, with PT and international normalized ratio (INR)remaining within normal limits. Local measures, such as placing gel foam in the bony cavity and applying a pressure dressing, effectively controlled bleeding from the cavity, and no postoperative bleeding was observed. A literature review of reported cases of FVII deficiency in patients who underwent minor and major oral surgeries based on the PubMed database is also presented.

This case highlights the importance of careful surgical and anesthetic management in patients with congenital FVII deficiency in a rare case of ossifying fibroma of the mandible.
