Squamous Cell Carcinoma Lesion at Fibula Free Flap Donor Site Following Mandible Reconstruction in a Known Case of Carcinoma Mandibular Alveolus: Case Report and Review of Literature

A 49-year-old male with a non-healing ulcer over the right lower mandibular alveolus was diagnosed with poorly differentiated squamous cell carcinoma of mandibular alveolus (T4aN1Mx). The patient was electively posted for composite resection with reconstruction using fibula free flap. In the postoperative period (week 6), the patient developed an ulcer on the free fibular flap donor site along the edges of skin graft. Biopsy taken from the ulcer edge demonstrated squamous cell carcinoma. This article discusses the probability of iatrogenic seeding of tumor cells intraoperatively and reviews similar case reports in the literature. We additionally discuss factors other than iatrogenic causes.
