<p>Health reports show that social and healthcare professions experience a&#xa0;disproportionately high level of work incapacity due to mental disorders. This also affects employees in hospitals, including those in social services. An online survey was conducted among all social services at 2503 hospitals in Germany on work-related psychological stress and strain at the turn of the year 2023/2024. The data were based on the Federal Statistical Office’s list of hospitals and preventive or rehabilitation facilities. The response rate was 24.7% with 619 social services. The measurements were carried out using the German version of the Copenhagen Psychosocial Questionnaire (COPSOQ). For each item, the means from the COPSOQ database were used to compare the means of the social services in hospitals with a)&#xa0;the social work occupational group and b)&#xa0;all occupations in Germany. Hospital social services stated higher quantitative requirements compared to the reference groups of social work (<i>p</i> &lt; 0.001; d = 0.611) and all professions (<i>p</i> &lt; 0.001; d = 0.511). Hospital social services also reported higher emotional demands than social work group (<i>p</i> &lt; 0.01; d = 0.108) and all professions (<i>p</i> &lt; 0.001; d = 1.173). They also exhibited burnout symptoms more frequently than the social work group (<i>p</i> &lt; 0.01; d = 0.145) and all occupations (<i>p</i> &lt; 0.001; d = 0.205). In addition, they reported greater presenteeism than the social work group (<i>p</i> &lt; 0.001; d = 0.224) and all occupations (<i>p</i> &lt; 0.001; d = 0.138). The findings point to higher levels of psychological stress and strain in social services in hospitals. There is a&#xa0;need for stress reduction, burnout prevention, and workplace health promotion in hospital social services.</p>
