Efficacy of different high dose rate interstitial brachytherapy for tongue cancer: a systematic review

The study evaluates treatment outcomes of oral tongue cancer patients treated with different high dose rate brachytherapy (HDR-BT) treatment options, focusing on local control rate (LC), cause-specific survival (DSS), overall survival rate (OS), and late effects, comparing and summarising published literature on various HDR-BT modalities to inform treatment decisions.
MethodsUsing the PubMed search engine, a systematic review was conducted in the Medline database to identify studies that report outcomes of tongue cancer treatment using HDR-BT. The search yielded 95 articles. After screening, twenty-four articles comprising 783 patients were eligible and analysed. Thirty patients were treated with HDR-BT following surgery and external beam radiotherapy (EBRT), 76 had adjuvant surgery and HDR-BT, 166 received boost HDR-BT following EBRT, 206 had HDR-BT following chemotherapy and EBRT, and 305 were treated with sole HDR-BT.
ResultsThe prescribed dose and fractionation schedule varied with the treatment modality, with the highest (median dose) in sole HDR-BT (40–60 Gy in 9–10 fractions) and the lowest in HDR-BT in combination with EBRT and surgery treatment (18–20 Gy in 5–6 fractions). Toxicity and late bone and soft tissue necrosis effects occurred in 70 (8.6%) patients. LC, RC, DSS, and OS rates were promising but varied among HDR-BT treatment modality and patient disease conditions.
ConclusionHDR-BT, when used alone or in combination with other treatments, provides significant advantages in managing tongue cancer, provided patients are carefully selected.
