The burden of emotional and behavioral problems is enormous. As treatment and prevention have so far failed to reduce their prevalence, recent policy documents emphasize the importance of healthy environments and the need for a broader, more integrated, and long-term prevention approach. This article outlines the preconditions for effective prevention, in which I focus on conditions that are not yet sufficiently clear, such as financing, management, complexity, evidence dilemma, long-term, structurally financed anchoring in relevant institutions such as maternity care, family support, education, and care social security. The required investments are enormous, but returns are extensive and land in many domains.
