<p>Sugarcane (<i>Saccharum</i> spp. hybrid), a tropical grass cultivated across more than 100 countries, produces the highest biomass among major crops. However, its productivity is increasingly challenged by diseases, many of which are intensified by climate change. This review critically examines how environmental stressors high humidity, narrow temperature ranges, waterlogging, drought, and warm winters amplify the spread of fungal, bacterial, and viral pathogens. It also highlights the role of sucking pests in transmitting viruses and the emergence of more virulent pathotypes that undermine elite varieties. To address these threats, the review outlines integrated mitigation strategies, including sustainable crop and soil management, disease-free seed systems, precision agriculture, and climate-resilient cultivars. The importance of predictive modeling is also underscored for timely disease forecasting and control. Together, these insights offer a forward-looking framework for managing sugarcane diseases amid changing climatic conditions.</p>
