Application of preoperative recombinant thrombomodulin in emergency laparoscopic cholecystectomy for acute cholecystitis with DIC

Recombinant thrombomodulin (rTM) has emerged as a promising therapeutic option for sepsis-associated disseminated intravascular coagulation (DIC). We report a case of acute cholecystitis complicated by DIC that was successfully managed with perioperative rTM administration in combination with laparoscopic cholecystectomy. The patient, a woman in her sixties, presented with acute cholecystitis and concomitant DIC. rTM therapy was initiated prior to surgery, and emergency laparoscopic cholecystectomy was performed with the application of the Pringle maneuver. The patient rapidly recovered from DIC without perioperative hemorrhagic complications, and this case suggests the feasibility and safety of a therapeutic strategy combining preoperative rTM administration followed by emergency laparoscopic cholecystectomy with the Pringle maneuver.
