<p>Hepatic sarcomatoid carcinoma (HSC) is a rare and highly aggressive subtype of primary liver cancer with no established treatment for unresectable cases. We report the case of a 71-year-old woman with unresectable HSC who achieved complete remission following carbon-ion radiotherapy. The patient exhibited a favorable response to primary tumor and lymph node metastases, with no recurrence observed after 17&#xa0;months. This case highlights the potential efficacy of heavy-ion therapy as a promising alternative for managing HSC.</p>
