After excision of cutaneous tumors in the forehead and cheek region, surgeons are faced with predominantly round or oval defects in an anatomically and aesthetically demanding area. Depending on the size and depth of the defect, location and tissue laxity, primary closure by advancement, various local flaps (advancement, rotational transposition-and island flaps) as well as split- or full-thickness skin grafts can be used. Local flaps provide color- and texture-matched tissue and allow scar placement along relaxed skin tension lines, facilitating tension-free closure and inconspicuous scars. In contrast, skin grafts represent an important option for larger defects or when local flaps are not possible, but their use in the facial area is limited by potential contraction, pigmentary changes and differences in surface quality. A stepwise reconstructive algorithm, careful preoperative planning and atraumatic suturing techniques are essential to prevent functional impairment and to achieve an optimal aesthetic outcome in forehead and cheek reconstruction after tumor surgery.
