Three frozen section methods are currently available: on the one hand, the conventional frozen section method, which is still very widespread and can still be found in many practices and academic centers. Of all three methods, the fastest is ex vivo confocal microscopy, which is about twice as fast as conventional frozen sectioning. What must be emphasized with this method is that only one person is required to operate this device, and the operator can also perform the quick cut. The principle of this method is that tissue is processed in fluorescent fluids (fast green and acridine orange). The tissue is then scanned with a laser that emits two wavelengths (488 and 638 nm). The light reflected from the tissue is analyzed and converted into an electronic image, which is displayed on a screen and imitates an hematoxylin and eosin (H&E) image. This offers the possibility of zooming into this image and enlarging it as with conventional light microscopy. With this method, particular care must be taken to ensure that the tissue samples are always processed in an oriented manner. In addition, the tissue must be placed vertically on the slide, especially in dermatopathology, so that the image quality is optimal. Possible sources of error could be incorrect staining and nonoptimal placement of tissue in the device. According to literature, there is a high concordance between diagnosis of ex vivo confocal diagnostic and the final H&E histology.
