Topical therapy in dermatology has significantly advanced in recent years, providing new hope for patients with various skin conditions such as atopic dermatitis (AD), alopecia areata (AA), and vitiligo. In the treatment of AD, basis therapy, topical steroids, and calcineurin inhibitors play a central role. Basis therapy focuses on strengthening the skin barrier, while topical steroids are used for short-term treatment of inflammation, and topical calcineurin inhibitors offer an alternative, especially for sensitive skin areas. New developments like topical PGE4 inhibitors and Janus kinase (JAK) inhibitors expand treatment options and offer promising results for AD patients. For AA treatment, intralesional corticosteroid injections and contact immunotherapy are important options. The topical application of JAK inhibitors such as tofacitinib and ruxolitinib has also attracted research interest, although their effectiveness varies in studies. The approval of the first vitiligo-specific therapy, ruxolitinib cream, by the European Medicines Agency (EMA) in April 2023 marks a significant milestone. Overall, these developments demonstrate the transformative potential of topical therapy and offer new hope for patients. Further research is needed to confirm the long-term effectiveness and safety of these new treatment options and fully realize their potential.
