<p>There is no equity in healthcare provision in Germany, and this includes oncology. This is in part due to structural disadvantages resulting from social determinants of health, which lead to sometimes dramatic differences in cancer incidence and survival rates. It is also due to differences in representation in medical research, resulting in poor applicability of medical evidence to underrepresented groups. Finally, certain groups or diseases are so unattractive for medical care in the German healthcare system that there is systematic underprovision. Efforts to improve equity in oncology care in Germany should address structural disadvantages on the one hand and increase the visibility and representation of previously marginalized groups in research and care on the other.</p>
