<p>This study examines the impact of SNPs in cytochrome P450 2C19, P-glycoprotein, and P2Y12 receptor genes on clopidogrel response in coronary patients. We conducted a cross-sectional study involving 135 acute coronary syndrome patients on clopidogrel. Platelet reactivity was assessed using the VerifyNow® P2Y12 test, with clopidogrel resistance defined as PRU ≥ 208. Genotyping was done by PCR–RFLP. Clopidogrel resistance was observed in 33% of patients. The <i>CYP2C19*2</i> polymorphism was associated with poor response (GA vs. GG; <i>p</i> = 0.048, OR 2.63, 95% CI 1.01–6.87). The <i>CYP2C19*</i>17 polymorphism was more common in responders (CT/TT vs. CC; <i>p</i> = 0.018, OR 0.35, 95% CI 0.14–0.87). No significant association was found for <i>ABCB1 (C3435T), P2Y12 (T744C),</i> or <i>ITGB3 (rs5918)</i> polymorphisms (<i>p</i> &gt; 0.05). Our results constitute preliminary proof for the implication of genetic polymorphisms in the response of platelet to clopidogrel in Tunisian coronary patients.
