<p>This systematic review and meta-analysis evaluates the anti-inflammatory effects of curcumin in cancer patients, focusing on randomized controlled trials (RCTs) that assessed its impact on inflammation-related outcomes. A comprehensive literature search was conducted across databases, including PubMed, Web of Science, Scopus, and Cochrane Library up to June 2024. Six RCTs containing 10 RCTs assessing the effects of curcumin on inflammatory biomarkers in cancer patients, such as myeloma, colorectal, and oral cancers, were included. Data were extracted and analyzed using STATA software to calculate weighted mean difference (WMD) with 95% confidence intervals (CIs) and assessed heterogeneity between the included studies. The meta-analysis demonstrated that curcumin intake significantly lowers the levels of inflammatory markers, including nuclear factor kappa B (NF-κB) (WMD − 0.46, 95% CI [− 0.82 to − 0.11]; I<sup>2</sup> = 0.0%, <i>p</i> = 0.61), and vascular endothelial growth factor (VEGF) (WMD − 62.39, 95% CI [− 114.33 to − 10.44]; I<sup>2</sup> = 100%, <i>p</i> &lt; 0.001). Additionally, subgroup analysis revealed substantial decreases in C-reactive protein (CRP) (WMD − 2.35, 95% CI [− 4.03 to − 0.67]; I<sup>2</sup> = 59.16%, <i>p</i> = 0.12) when curcumin was given alone and in VEGF (WMD − 73.29, 95% CI [− 132.89 to − 13.68]; I<sup>2</sup> = 100%, <i>p</i> &lt; 0.001) when combined with other therapies. In addition, treatment lasting ≥ 2 months was linked to lower levels of NF-κB (WMD − 0.53, 95% CI [− 0.92 to − 0.15]; I<sup>2</sup> = 0.0%, <i>p</i> = 0.73). Curcumin had no statistically significant effect on TNF-α, CRP, or IL-6 levels. These data show that curcumin may have anti-inflammatory properties in cancer treatment; however, the effects may be context-specific. Larger, more rigorous trials are needed to validate these findings and improve curcumin’s therapeutic use.</p>
