<p>Breast cancer is the commonest cancer among Indian women (age-adjusted rate: 25.8/100,000; mortality: 12.7/100,000). While neoadjuvant chemotherapy is vital in treatment, it carries cardiovascular risks, including left ventricular dysfunction. Cardiac biomarkers like troponins can detect early cardiotoxicity, but few Indian studies address preclinical prediction. Thirty-seven postmenopausal breast cancer patients planned for neoadjuvant chemotherapy were enrolled. Left Ventricular Ejection Fraction (LVEF) and psychological stress (PSS, PHQ-9, GAD-7) were assessed before and after 7 cycles of chemotherapy. Serum high-sensitivity cardiac troponin I (hscTnI) levels were measured with CLIA kits. Post-neoadjuvant chemotherapy (NACT), reductions were observed in BMI (<i>p</i> &lt; 0.01) and haemoglobin (<i>p</i> &lt; 0.01) with increment in fasting blood sugar and lipid profile (<i>p</i> &lt; 0.01). Left ventricular ejection fraction (LVEF) decreased and hscTnI increased (<i>p</i> &lt; 0.001) post-NACT. Psychological distress increased with elevated scores in PSS (<i>p</i> = 0.001), PHQ-9 (<i>p</i> &lt; 0.001), and GAD-7 (<i>p</i> &lt;  0.001). Inverse associations of LVEF were observed with hs-cTnI (<i>r</i> = −&#xa0;0.580, <i>p</i>  &lt; 0.001) and psychological distress scores, PSS (<i>r</i> = −&#xa0;0.397), PHQ-9 (<i>r</i> = −&#xa0;0.334), and GAD-7 (<i>r</i> = −&#xa0;0.432). Post-NACT, 30.5% of the patients developed asymptomatic mild ‘Cancer Therapy Related Cardiac Dysfunction’ (CTRCD) (<i>p</i> = 0.006) according to the ESC guidelines. Patients with CTRCD had lower LVEF (<i>p</i> &lt; 0.001) and higher hscTnI levels (<i>p</i> &lt; 0.001). These observations underscore the importance of monitoring cardiac and psychological health during breast cancer treatment.</p>
