Genetic Polymorphisms in XRCC1 Gene as Predictive Biomarkers for Radiation Toxicity in Head Neck Cancer Patients Treated with Chemoradiotherapy

Head neck cancer (HNC) ranks as the sixth most common cancer globally, in India, where tobacco use are very common, HNC is the second most common cancer among men, contributing to low five-year survival rates ≤ 40%. Radiotherapy, often combined with surgery and chemotherapy, is a primary treatment modality, but 10–70% of patients experience acute or late radiation-induced toxicity, significantly affecting quality of life. This study investigates the association of single nucleotide polymorphisms (SNPs) in DNA repair genes XRCC1, ERCC4 (XPF), and XRCC4 with radiation-induced toxicity in 55 HNC patients from central India. Patients were treated with volumetric modulated arc therapy (VMAT) and assessed for toxicity using the Radiation Therapy Oncology Group criteria. Genomic DNA was extracted and SNPs analyzed using PCR-RFLP. Acute and chronic toxicities were recorded, with significant grade 3 or higher toxicities observed in 73% of patients during the acute phase and 75% during the late phase. The study found significant associations between certain XRCC1 SNPs (rs25489 and rs915927) and high-grade acute toxicities such as trismus, dermatitis, and xerostomia. The data suggest that genetic variations in DNA repair genes influence the susceptibility to radiation-induced toxicity in HNC patients.
