<p>This retrospective single-centre study investigates the transplant-free survival (TFS) and treatment response in patients with T-cell Acute Lymphoblastic Leukemia (T-ALL). Thirty-three patients diagnosed with T-ALL were evaluated for demographic variables, response to induction chemotherapy, measurable residual disease (MRD), relapse rates and long-term survival without hematopoietic stem cell transplantation. MRD &lt; 0.01% post-induction correlated significantly with superior TFS (<i>p</i> = 0.031). Age and sex were not significantly associated with survival. The findings underscore the prognostic value of MRD and early treatment response in guiding T-ALL management without necessitating transplantation under vigilance.</p>
