IgD Myeloma Mimicking Lymphoma: A Diagnostic Pitfall

A 70-year-old male with fatigue and pancytopenia was found to have IgD-lambda multiple myeloma. Initial bone marrow morphology showed blastoid cells, misleadingly suggestive of lymphoma. Diagnosis was established by immunophenotyping (plasma cell phenotype) and cytogenetics, highlighting a critical diagnostic pitfall where cytomorphology contradicted the immunophenotypic findings.
